HTH Medical - Member Claim Form -

WAMS provides health insurance for Sonangol Scholarship Recipients through HTH Worldwide Insurance Services.

Insurance for students is renewed each year on September 1st.  Each September/October students will receive a new medical insurance ID card.  Please keep this card and carry it with you at all times.

When you receive your new card please register at hthstudents.com using your individual insurance certificate number listed on your identification card.  Once you are registered you will be able to do the following:

• Review details of your insurance plan
• Search for best medical care providers
• Track the status of submitted claims

To register follow the steps below:

1. Go to www.hthstudents.com  and click on “sign in”
2. Fill in your e-mail address and click “No, I’m signing up for the first time” and then , “Sign In”
3. Enter your certificate number (on the from of your ID card), name and re-confirm your e-mail address
4. Review and accept the brief “Site Use Agreement”
5. Create a password (must contain letters and at least 1 number)

• Students are eligible to receive prescription medication discounts in conjunction with the MedCare Pharmacy Network, which has an extensive, countrywide pharmacy network.
• In order to receive a discount, present your card to your pharmacist when ordering your prescription.
• To find a pharmacy provider in your area that accepts the card, log on to the MedCare website at www.medimpact.com and search the MedStandard Gold Network by entering your certificate number on the from of your card or call 1-800- for MedCare Customer Service.
• For reimbursement of covered prescriptions, members must submit a claim form to the claims administrator at the address listed on the front of your card.

Please use the HTH claim form above to submit claims if your health care provider does not submit claims for you.
